Photo Release Form

I hereby waive individually, and on behalf of all attendees, minor, or otherwise from whom I am purchasing tickets (“Participant(s)”), and release the Celiac Disease Foundation and its officers, directors, employees, agents, and insurers from any and all claims, demands, actions, and causes of action, which may arise out of, or in connection with, Participant(s) attendance at the Gluten-Free EXPO. In addition, Participant(s) agree to defend and hold the Celiac Disease Foundation harmless from and against any and all claims, liabilities, expenses, damages, losses, causes of action, and suits (including, without limitation, attorneys’ fees and costs) arising out of, or any way related to, Participant(s) attendance at the Gluten-Free EXPO.

Participant(s) agree that photographs, videos, and testimonials taken of Participant(s) or other materials created by Participant(s) (“Media”)and submitted to Celiac Disease Foundation shall become the property of the Celiac Disease Foundations, at its discretion, for any publicity or marketing purposes, and Participant(s) hereby irrevocably consent and authorize such use without restriction. Photographs, videos, testimonials, artwork, etc. of Participant(s) help explain our mission through brochures, newsletters, annual report and website and other promotional outlets including television, newspaper, magazines articles and social media sites including Facebook. In addition, Participant(s) waive the right to inspect or approve the finished product, including written or electronic copy, wherein Participant(s) likeness appears.  Additionally, Participant(s) waive any right to compensation arising or related to the use of Media. Participant(s) hereby hold harmless and release and forever discharge the Celiac Disease Foundation from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I have read this release before signing below and I fully understand the contents, meaning, and impact of this release. I warrant that I am of legal age and competence to give a valid release on behalf of Participant(s).
